HOURS: 40 hours per week

1. PRIMARY PURPOSE
The Program Specialist provides quality, comprehensive intake and case management services to individuals and their families seeking services for needs relating to aging and disability. Maintains a body of knowledge and experience relevant to providing situationally appropriate and sensitive services to clients. Provides intake services to all Community Action Team (CAT) clients. Operates as part of the CAT Human Investment (HI) team.

2. ESSENTIAL DUTIES AND RESPONSIBILITIES
• Provides support and direction to callers requesting information or services within the Senior Programs purview. Connects clients to appropriate, relevant resources in a timely and empathetic manner.
• Conducts intake services over the phone and in-person. Sets up access to services, records client information, and provides ongoing support to clients and caregivers.
• Advises and supports team members in active case management activities on behalf of clients. Provides direction to care coordination team to deliver services efficiently and effectively.
• Conducts site visits at nutrition program meal sites in order to assess program compliance and provide necessary guidance and support to onsite staff.
• Assists with planning and implementation of ongoing program activities and special projects to further services in the senior programs area.
• Attends meetings and participates in partnership activities and events with relevant stakeholders and community partners throughout the CAT service area.
• Performs entry of relevant data into online database, OPUS, and other appropriate software applications, for purposes of tracking, managing, and maintaining program records. Thoroughly documents applicant eligibility status, client-specific details, income information, statistical data for reports, and other relevant information to support compliance with program regulations.
• Conducts home visits, as needed.
• Conducts welfare checks, as requested.
• Substitutes for meal delivery staff when needed.
• Occasional participation on senior-related boards across the county, including those for the AAA, the area’s Senior Centers, and the Multi-Disciplinary Team (MDT) for senior abuse/neglect.
• Maintains a working knowledge of program policies, rules, and performance standards.
• Processes reports and provides regular status updates, as needed.
• Serves as a member of the HI team, contributing ideas and suggestions to improve program functioning and service delivery.
• Supports data entry for other programs, as appropriate.
• Ensures and maintains strict confidentiality of all sensitive client and program data.

3. OTHER DUTIES AND RESPONSIBILITIES
• Other duties as assigned by the Executive Director, Human Investment Director, or Program Manager.

5. MINIMUM QUALIFICATIONS AND EXPERIENCE
• High school diploma or equivalent required.
o Bachelor’s degree in social services, sociology, public health, business administration, human health and development, or another, relevant field of study preferred, not required.
• Knowledge of program issues related to veterans and homelessness preferred, not required.
• Bilingual speaking and writing skills (English/Spanish) preferred, not required.
• Minimum one (1) years’ experience performing similar duties in a similar role, such as a nonprofit program coordinator, case manager for seniors, etc.
• Strong proficiency in Microsoft Suite (i.e., Excel, Word, PowerPoint, etc.).
• Strong proficiency in using database technologies.
• Proven experience analyzing problems and identifying viable solutions to achieve effective outcomes.
• Ability to work effectively and cordially with community members with diverse life experiences and backgrounds.
• Ability to become certified as an Options Counselor within one year of hire.
• Ability to become I&R AIRS-AD certified within one year of hire.
• Must have access to reliable transportation.

9. ATTENDANCE
• Typical schedule is Monday-Friday
• Hours are generally 8:30 AM – 5:00 PM with occasional need for weekend or off-hours support for events, trainings, etc.

10. SAFETY
