Please bring the following when you turn in your housing intake forms:
- Photo ID for all adults on the application
- Last 30 days proof of income from all adults on application
- Social Security cards for everyone on the application
- Bank statement
- Current year Social Security Award letters, if on Social Security
- Any notices given by landlord

Housing Programs
Each of these programs reflects Community Action Agencies’ commitment to addressing homelessness with a comprehensive, client-centered approach. They are often coordinated with other local services and resources to ensure a holistic response to housing instability. Collaboration with local government, non-profit partners, and community stakeholders is key to the success of these programs, ensuring they meet the specific needs of the community they serve.
Permanent Housing Program
Permanent Housing Program is designed to provide ongoing rental assistance, case management, and supportive services for low-income individuals who are chronically homeless. This program aligns with the Housing First model, ensuring that those with the most severe housing needs receive priority.
Funding and Eligibility: Funded through federal initiatives like HUD’s Continuum of Care (CoC) program and state/local grants, this program targets chronically homeless individuals, often with disabilities or other complex needs.
- Ongoing rental assistance to reduce the financial burden.
- Comprehensive case management focusing on the individual’s holistic needs.
- Supportive services including counseling, healthcare, job training, and financial literacy programs.
- Assistance with obtaining necessary documentation and benefits.
Rapid ReHousing Program
The Rapid ReHousing Program offers timely rental assistance and supportive services to homeless individuals and families, helping them transition quickly from homelessness to permanent housing.
Funding and Eligibility: This program is often supported by federal grants like the Emergency Solutions Grants (ESG) program, targeting individuals and families facing immediate homelessness.
- Short to medium-term rental assistance.
- Case management focusing on rapid rehousing and stabilization.
- Employment assistance and job readiness training.
- Access to health services and child care resources.
Supportive Services for Veteran Families (SSVF)
Funding and Eligibility: Funded through the VA’s SSVF program, it targets veterans facing homelessness, offering a bridge to stable housing and self-sufficiency.
The SSVF program is dedicated to assisting eligible low-income veterans and their families who are homeless or at risk of becoming homeless in obtaining or retaining permanent housing.
- Rental assistance and utility payments.
- Case management with a focus on long-term stability.
- Assistance with VA benefits and other public benefits.
- Legal aid, credit counseling, and supportive services tailored to veterans.
One-Time Deposit and Rental Assistance
The One-Time Deposit and Rental Assistance program provides limited, one-time financial assistance to low-income individuals and families who are homeless or at risk of homelessness, assisting with deposits or rent to prevent homelessness.
- One-time financial assistance for security deposits or rent.
- Financial counseling and budgeting assistance.
- Referrals to other supportive services and housing resources.
Eviction Prevention/Deposit Assistance
For low income individuals and households who are homeless or at risk of being homeless and meet program eligibility requirements. Income must be more than rent amount and monthly expenses. Cannot prolong an eviction.
Case Management
Preparation and support for securing and/or maintaining safe and secure rental housing. Provide households with individualized supports which may include, but are not limited to; budgeting, support identifying housing, referrals/ connection to wrap supports, mainstream and local community resources.
Funding Streams and Partnerships
We participate in the following programs as funding allows: EHA, ERA, ESG, HSP, SHAP.
Coordinated Entry (CE) is a system that allows for coordinated entry into a local homeless services system, as well as coordinated movement within and ultimately exit from the system. Coordinated Entry increases the efficiency of a homeless assistance system by standardizing access to homeless services and coordinating program referrals.
The Rural Oregon Continuum of Care (ROCC) is committed to sustaining a CE system that will help to advance our goals of helping households quickly access appropriate services to address housing crises, increasing exits to housing, decreasing length of time homeless, and reducing returns to homelessness.
