The Options Counseling program provides short-term counseling services to help you make informed choices about your long-term care needs. We support older adults and those acting on their behalf with information about immediate and long-range care options.

An Options counselor can guide you through questions such as:

  • Can I live in the community safely and independently?
  • What services and supports are available for me in my home?
  • Can I go to a rehabilitation facility or nursing home temporarily and then return to my home?
  • What services, assistive devices, or home modifications are available to support me in my home?
  • What funding resources may be available for supports and services?

How to Access Our Services

Contact Us

Please contact our Options Counselors for more information or to make an appointment. We can visit you in your home, meet with you in a nursing home or rehabilitation facility, or communicate via telephone and email.

Initial Contact and Follow-Up

An Options Counselor will contact you to discuss any immediate needs over the phone and set up initial face-to-face meetings, typically within two weeks.
